Patient Centered Medical Home (PCMH) continues to be a dominant model at health centers around the US. Through PCMH, health centers are providing comprehensive, quality care from a team of providers working together for the patient’s whole body health. A mobile patient like an agricultural worker presents a big challenge in successfully executing PCMH because the patient may be in the service area for a very limited time and may encounter numerous other barriers to health care due to mobility, poverty, language, culture, and more.
