Pharmacists can play a key role in the care team at community health centers to better patient care and increase health equity. In this webinar, we will look at how one community health center utilizes pharmacists to best serve their community. We will consider the inclusion of pharmacists in issues such as emergency response to disaster and other emerging health concerns where pharmacists can strengthen patient care. Participants will also learn about strategies and promising practices to increase communication and collaboration between pharmacists and other members of the care team like primary care providers and community health workers.

Alyssa Puia is a Clinical Pharmacist at Holyoke Health Center, a community health center serving the communities in and around Holyoke for over 50 years.  In her role, she is utilizing her knowledge of pharmacy and the Spanish language to provide ambulatory care services to underserved patient populations. She is involved in clinical programs including transitions of care (TOC), medication therapy management (MTM) and collaborative drug therapy management (CDTM) for hypertension and diabetes. She is also passionate about public health and vaccines, coordinating the Holyoake pharmacist-led immunization program and related grant projects to improve the immunization rates in our community.
